GLP-1 for longevity: what the science says, and what the headlines skip.

THE 30-SECOND ANSWER

GLP-1 medications are the most rigorously studied weight and metabolic drugs of the last decade — 15-25% weight loss, 20% fewer heart attacks and strokes, proven in trials with tens of thousands of people. The longevity angle is newer and more early-stage, but the metabolic and anti-inflammatory effects are real and worth asking a clinician about.

What it is

GLP-1 receptor agonists (semaglutide, tirzepatide) mimic a gut hormone your body makes after eating. They slow digestion, regulate blood sugar, and quiet appetite. Originally for diabetes, they've turned out to do far more.

What the evidence actually shows

STRONG

15-25% body weight loss sustained over 72 weeks — the STEP and SURMOUNT trials.

STRONG

20% reduction in heart attacks and strokes — the SELECT trial, 17,604 patients, independent of diabetes.

STRONG

24% slower kidney disease progression — the FLOW trial, stopped early because the benefit was clear.

EARLY

Reduced systemic inflammation (CRP) — the mechanism most tied to the longevity conversation. Emerging human data.

ANIMAL

Neuroprotective effects — promising in animal models, human trials underway.

The longevity question — honestly

Here's what we don't know: whether the longevity benefits are a direct effect of the drug or simply the result of resolving obesity and inflammation. Whether "microdosing" for metabolic health in people who aren't overweight actually delivers. The long-term effects of years of use. These are open questions — interesting enough to explore with a clinician, not settled enough to treat as fact.

Who should be careful

GI side effects are common early. Without resistance training and adequate protein, a meaningful fraction of weight lost can be muscle. There's a boxed warning regarding thyroid tumors, and it's contraindicated with a history of medullary thyroid carcinoma or MEN 2. This is a conversation for a clinician who knows your history.

The sourcing thing matters here too

The compounded semaglutide window has largely closed with the shortage resolved. If you're going to use a GLP-1, using an FDA-approved product through a licensed provider with pharmaceutical-grade sourcing isn't just safer — increasingly, it's the only compliant path.
